Evidence to objective #5

 

 

During an initial intake with a new patient coming to our facility, we usually sit down with the patient one on one to get to know them and their cardiac/pulmonary situation.  In the intake we sit down the patient and review the following information with them....

  • Medication history—time and days meds are taken as well as dosages

  • Symptoms they experienced during their cardiac event

  • Family medical history

  • Personal medical history

  • Orthopedic limitations to exercise

  • Blood pressure measurements

  • BMI and waist measurements

  • Prior experience with exercise

  • Goals they would like to fulfill during their time in our facility*

After much conversation with them and discussing their goals during the 12 weeks at the facility, we explain to them what the Miriam has to offer them and how our services will benefit them and help them accomplish their new goals.  

 

 

To the left are two example initial intake sheets that we use during an initial intake with a patient.  Patient information sheets like this are all slowly turning electronic, in which the facility is currently in the transition stages of going paperless.  With that said, the Miriam has created a patient portal where their patients can answer questions and fill out questionnaires via the computer.
